Our eyes are our most important sensory organ. They help us lead through life. And the more clearly we see the more life has to offer. Visual clarity depends on how optical impressions arrive at our retina, the inner surface at the back of the eye. The cornea, the lens and the vitreous body blend light rays entering  the eye to focus them on a particular point on the retina. If this focal point occurs either in front of or behind the retina, unclear vision results. Eye doctors measure this defect in diopters.

Three main types of visual defects

Near-sightedness
In a near sighted eye, incoming light rays form an image in front of the retina because the cornea is curved too strongly or the eyeball is too long. Result: Objects at a distance appear blurred and out of focus.

Far-sightedness
In a far sighted eye, images from behind the retina, because the cornea is curved too weakly or the eyeball is too short. Result: Close objects appear blurred.

Astigmatism
In an astigmatic eye, the cornea has an uneven curvature. Instead of converging at one focal point, light rays merge in a line infront of or behind the retina. Astigmatism can also occur in combination with near-sightedness and far-sightedness. Result: A distorted view of objects.

 
Before, During, After

Although spectacles and contact lenses improve vision, they are also a constant inconvenience of daily life. However despite millions of successful LASIK treatments performed worldwide, many people are still unsure if the treatment is right for them. To help you understand the various steps, we have provided a brief guide on the laser vision correction from the first consultation to the final check up.

Consultation and proper examination
An extensive consultation and examination with your eye doctor will determine whether you could benefit from laser vision correction. During the consultation, your doctor will advise you on any possible risk or side effects of the procedure and answer any questions you may have. Laser vision correction is suitable for most people who have near or far sightedness or astigmatism. However there are important exceptions, which your doctor will explain to you during your consultation. Presbyopia (an age-related loss of the ability to focus at various distances) can generally not be corrected in all the cases.

Treatment day
- First you will receive anaesthetic eye drops.
- After a thorough check of the laser settings, your eye doctor will begin treatment.
- You will lie relaxed on a patient bed. An eyelid holder will ensure that your eye remains open during treatment.
- You will be asked to focus on a blinking light throughout treatment.
- The entire treatment normally takes no more than a few minutes.

And after that
- You will be given eye drops and an eye guard for use at night.
- Spend the rest of the treatment day relaxing.
- Follow your eye doctor's instruction during the healing process.
- Your eye doctor will check the results of your treatment during a follow up examination the day after treatment.
- The final result usually stabilizes within about four to six weeks following treatment.

LASEK the alternative to LASIK

LASEK stands for Laser Epithelial Keratomileusis. as opposed to LASIK, which stands for  laser in-situ  keratomilieusis
This new form of refractive eye surgery is used to treat near-sightedness, far sightedness, and astigmatism. It is an evolution from an earlier version known as PRK, or Photorefractive Keratectomy.

LASEK and PRK are an alternative to LASIK especially in cases where the cornea is very flat or thin.

The subtle difference
LASEK and PRK differ from LASIK in one subtle but important detail: With LASEK and PRK, the cornea is not cut-instead the uppermost layer of the cornea (the epithelium) is temporarily removed

Advantages
The advantage of LASEK and PRK is that it doesn't require creation of a corneal flap. After treatment, a therapeutic contact lens is applied for three to five days to promote the healing process.

Disadvantages
The disadvantage of LASEK and PRK is that the healing process takes longer and can involve somewhat more pain or discomfort than lasik. However this can typically be treated with mild pain medication.

Who is a good candidate for LASEK
  •        Patients who have thin corneas who would otherwise not be good candidates for LASIK.
  •        Patients with large pupils who would not be good candidates for LASIK, because in these patients larger treatment zones are required and LASEK  allows more tissue to be spared in such patients.


 
 
LASIK - the procedure

Step 1: After your eye has been numbed with "eye drop" anaesthesia, an instrument called eye lid speculum will be positioned to hold your eye lids open. You will remain awake and comfortable throughout the procedure.

Step 2: A small suction ring will be placed around the cornea which serves as a platform for the microkeratome.

Step 3: The microkeratome separates the surface layer of the cornea (about 160 microns) and the corneal flap is folded back. you will be asked to look at the target light while the excimer laser   reshapes the corneal tissue.

Step 4: A clicking sound can be heard as each microscopic layer of tissue is vaporized. the process will last from seconds to minutes, depending upon the amount of correction required.

Step 5: The corneal flap is then placed back into its original position and allowed to dry for few minutes.

Step 6: You will be given eye drops, and your eye may be shielded for protection. Vision will probably be a little blurry for 12-24 hours. It is advisable to relax for the rest of the day.

 
LASIK - Am I a suitable candidate

In general, the ideal candidate for LASIK is one who is over 18 year of age and has a healthy cornea. Candidate must have a stable refractive error. Candidates with ocular conditions like unstable refraction, keratoconus (conical shape of cornea) and any active or recurrent ocular disease etc. are not considered for this procedure. People with certain medical conditions or women who are pregnant may not be good candidates for LASIK.

10) There's no guarantee of 20/20 vision Many people do achieve 20/20 vision after a LASIK surgery. But a good eye surgeon does not guarantee it. There can be complications sometimes. LASIK will correct only certain eye problems, so if you have any other problem that affects your vision, it remains after LASIK surgery.

How safe is the treatment?
Extremely safe. If the doctor and the patient co-operate during the course of the treatment, then there is hardly any chance for complications to arise.

How long is the treatment? Is it painful?
The entire process takes less than ten minutes in which the laser treatment itself takes only a minute or two.The treatment is not painful. No injections are necessary. However, for about one day after the treatment the patient might experience some pain.

Does the patient have to stay at the hospital?
No, the patient can go home immediately. Things that irritate the eye should, however be avoided for 2-3 weeks. For instance, in bright light one may need to wear sunglasses. The patient needs to put eye-drops for a few weeks after treatment.

Can both eyes be operated together?
Yes, Both eyes are normally treated together. When you get both eyes treated together, you save on time, and follow-up visits, and the visual recovery is also somewhat faster. However, this does carry some additional risk, so if you are more comfortable getting one eye treated at a time,that is also possible. Please discuss this issue with your doctor at My Eye.

Does LASIK cause any systemic difficulties?
No, LASIK does not cause any systemic difficulties. Its effect are restricted to the eye, and there are no documented cases, where any other effects have been shown.
